DOCUMENTATION: The following procedures shall be used to document disturbances within <br />the entire Reclamation Permit area. <br />I. All disturbances plus the type and responsibility for disturbances shall be reported to the <br />Division in the annual report due March 28 of each year. <br />A. A copy of the annual report will be provided to the Colorado State Board of Land <br />Commissioners (this is already done). <br />II. The annual report to the Colorado State Board of Land Commissioners, due July 15 of each <br />year, shall also report this same information, with updating as needed for additional <br />disturbances made between March and July. <br />A. A copy of this annual report will be provided to the Division (this is already done). <br />III. Disturbance by sand mining shall be distinguished from disturbances by oil and gas <br />development. <br />A. For sand mining disturbances the following general categories of land condition will <br />be indicated <br />1. Extraction and processing including transportation routes <br />2. Pre - revegetation reclamation (e.g. rough grading, fine grading, topsoiling) <br />3. Revegetation but not yet released from bond <br />4. Reclamation that has been released from bond <br />B. For oil and gas exploration and development <br />1. Exploration routes and disturbances <br />a. Disturbances that are more of a permanent nature shall be located on <br />the map and described in the annual report. <br />b. The annual report will also generally describe recovery aspects of older <br />exploration disturbances to document recovery rates and patterns. <br />2. Development disturbances <br />a. All roadways to oil and gas facilities shall be shown on the map. <br />b. The location of drill pads shall be shown on the map. <br />(1) only the outline of the drill pad will be shown <br />(2) nothing regarding what is on the drill pad will be shown <br />(3) drill pad and other oil and gas development reclamation shall <br />be shown on the annual report map <br />c. Photographic documentation shall not include features or aspects of <br />the oil and gas development that are considered proprietary. <br />(1) any photographs that show actual development facilities shall <br />first be reviewed by the office of the Colorado State Board of <br />Technical Revision - Coal Creek Resources - M- 1988 -044 September 2011 Page 4 of 5 <br />
