The emergency plan needs to be provided and accepted by the County Office of Emergency <br />Management (OEM), County Planning Engineering, Fort Lewis Mesa Fire District, and the <br />County Office of Emergency Management (OEM). <br />C. A Geotechnical Work Plan supporting the construction plans. <br />I. The Geotechnical Work Plan shall be prepared, bound, and sealed by a Professional Engineer <br />registered in the State of Colorado. When references are made to the Construction Plans in <br />the Geotechnical report„ or vice versa, the referenced document shall be identified with <br />specificity including report title, date, engineering firm, certifying engineer. <br />2. Estimate maximum quantity of rock and fill material to be stored at any one time and footprint <br />of staging area needed (proposed to be at base of new, unpermitted access road, near the La <br />Plata River) <br />3. Estimate quantity of fill and footprint that could impact wetlands, as indicated by the current <br />/ Work Plan <br />I 4. Clearly state how much fill above the 36 -inch culvert will remain, whether culvert will remain <br />or be replaced, and provide and inlet and outlet structure design specifications. <br />If buttresses are to be constructed as noted in Geotechnical Work Plan, provide design <br />specifications and details in Geotechnical Work Plan and Construction Plan Set. <br />6. The Work Plan needs to provide a definitive plan and design regarding how the retaining wall <br />will be removed and the road reconstructed (rather than putting responsibility on the <br />Contractor). Slope and rock stability data, including Factor of Safety, is needed to support the <br />design. <br />If Construction Plans ultimately show rock cuts, provide rock stability evaluation. <br />/5. <br />D. A Drainage Evaluation supporting the construction plans. <br />1. Provide a narrative and calculations that address drainage related to the new, unpermitted <br />access road. Critical items to address include: <br />a. Offsite flows <br />b. Onsite flows <br />c. Detention and sediment removal <br />d. Construction erosion control. Clearly show how wetlands will be protected throughout <br />phases of road, fill removal and replacement corresponding to Geotechnical Work Plan <br />e. Permanent erosion control (ie: revegetation in non- mining areas). Critical areas are the <br />bar ditch outfall and the 36 -inch culvert. <br />2. Ensure appropriate application of erosion control. ie: check dams with upper elevation of <br />lower dam being no lower than lower elevation of upper dam and need for check dams and silt <br />fences to be . fully entrenched. <br />3.
