The baseline log was obtained by using the fiber <br />optic cable in conduit cemented to the outside of the 9 5/8 -inch casing. <br />• Attachment B — Standard Annulus Pressure Test (SAP); (Part I MI) <br />• Attachment C — Fiber Optic Cable Conduit Pressure Test (conduit cemented to the outside of the <br />9 5/8 -inch casing) <br />The information provided in this submittal in combination with the AMSO's previous submittals of (1) <br />casing cement operations and evaluations for wells completed on the TM Pad (dated July 26, 2011), (2) <br />Step Rate Test (Well TM -1) and Resistivity and Porosity Logs (Well BH -1) dated August 15, 2011, and <br />(3) Casing Cement Operations and Evaluations Well T -1H (dated August 16, 2011) fulfills the logging <br />and testing requirements prior to receiving EPA authorization to inject specified in Appendix B of <br />AMSO's Underground Injection Control Program Permit. <br />
