,• <br />• <br />• <br />Pre - existing Damage Descriptions <br />The following terms are used to describe pre - existing structural and cosmetic damage. <br />Structural Damage <br />Minor — Minimal or noticeable cracks present in the foundation wall, load bearing walls or <br />support structures. Small cracks or noticeable expansion between bricks /mortar, and or chipping <br />or cracking of mortar between cinder blocks or bricks. <br />Major — Large cracking present in the foundation wall, broken floor boards, misaligned or bowed <br />foundation walls. Large cracks present in exterior stone or brick walls(cracking in stone/brick, <br />not just mortar). <br />Cosmetic Damage <br />Minor — Minimal or noticeable cracks present in wall or ceiling drywall or plaster. Small cracks <br />present in exterior stucco construction or concrete flat work. <br />Major - Water damage, chipping/peeling paint, missing or damaged exterior siding. Large cracks <br />present in exterior stucco construction or concrete flat work. <br />Copies of the inspection reports will be submitted to Joe Lamanna of Holcim US Inc. as well as <br />to each property owner. <br />Vibration Monitoring Program <br />The latitude and longitude of each residence or well was recorded during the pre -blast surveys. <br />Distance from the proposed blasting activities as well as age /condition of each structure was <br />taken in to account when deciding where to monitor blasting activities. <br />Matheson Mining Consultants, Inc. recommends placing vibration monitors at the following <br />residences in this order: <br />1) 13260 County Road 45 — Martellaro Residence <br />2) 565 Meadow Drive — Ray Residence <br />3) 500 High Peaks Ranch Road — McCafferty Residence <br />4) 1055 County Road 6 — Clifton Residence <br />Placing seismographs at these locations for all blasting activities will provide adequate vibration <br />coverage on all sides of the Holcim US Inc.
