An executed "Application Form for a <br />Revision" is included herein for the Division's review. <br />On September 8, 2011, the Division sent an email to TCLLC requesting updates, via a <br />Minor Revision, to various selected PAP information. Our response to the email is as <br />follows. <br />1. Fire Mountain Canal — We have been unsuccessful in finding any additional <br />drawings or plans for the 1983 Bridge Permit with the Fire Mountain Canal. <br />2. APCD Final Emissions Permits - Copies of the May 15, 1995 APCD Emission <br />Final Permits are attached and may be used to replace the Division's copies of the <br />"Initial Approval" permits in PAP Exhibit 2, Item 4. <br />3. Highway Access Permit - TCLLP was successful in finding a more legible copy of <br />the 1983 Department of Highways Permit for Access. A copy is attached for inclusion in <br />Exhibit 4, Item 5 of the PAP. As the Division is aware, the section of Highway 133 in <br />question has been replaced by the new Highway 133 located south of the North Fork of <br />the Gunnison River and is now known as the Bowie Road. <br />
