OBSERVATIONS <br />PERMIT #: M- 2006 -081 <br />INSPECTOR'S INITIALS: BMK <br />INSPECTION DATE: September 23, 2011 <br />1) The Division conducted a monitoring inspection of the site on 9/23/11. The site is a phased 112 <br />operation located approximately 2 miles west of Fowler Colorado. <br />2) Mr. Keith Baker the operator was present at the time of the inspection. The total permitted acres is <br />193, with 27 acres to be affected at any given time. Since the site was permitted in 2006, due to lack of <br />demand, the site has not been fully active. The only areas affected were approximately two acres along <br />the north east side of the permit area. The material was used to work on the irrigation ditch along the <br />same side. There were three small processed stockpiled material noted at the time of the inspection. <br />All the drainage from the area is captured with in the pit floor. <br />3) The current financial warranty ins the amount of $43,303.00 is more than adequate to cover the cost <br />of reclamation. <br />4) The entire permit area is fenced; a permit sign was posted at the access to the property. The operation <br />is in compliance with the approved permit. <br />Page 2 of 4 <br />
