30. Additional Monitoring: All applicants must review the parameters listed in Appendix A and Appendix B to this application and indicate whether any of <br />these pollutants are known or believed to be present. For every pollutant expected to be discharged, the applicant must briefly describe the reasons the <br />pollutant is expected to be discharged and report any quantitative data for the pollutant. See cover letter <br />• <br />31. CDMG Monitoring: Please submit a copy of summaries of all water quality monitoring data of outfalls or receiving streams for which data has been <br />gathered over the last five years for the mining site and which is required by the Colorado Division of Minerals and Geology. If not already submitted to <br />the Division, the plan which details the monitoring frequency, type, locations and method of analysis must also be submitted. If these data are <br />voluminous, please contact the Division. See cover letter <br />32. Bulk Storage: Do you have bulk storage of diesel fuel, gasoline, solvents or other hazardous materials on site? Yes If Yes. briefly list materials, <br />their quantity, measures taken to ensure that spills do not enter state waters and indicate their location on site sketch, Item 12. <br />See cover letter. <br />For renewal applications, list the date when the most recent Materials Containment Plan was submitted. <br />33. Activity Duration: When did the mining operation commence? 1962 Estimated life of the activity - Mining completed, reclamation in progress. <br />34. Pollution Prevention Plans: Please describe any pollution prevention or best management plans currently in place that could result in the improvement <br />of water quality. These could include solvent recycling programs, material containment procedures, education, etc. <br />See cover letter. <br />35. Historic Drainages: Does historic drainage exist at the site, which is not covered under a CDPS permit? No If so, please provide a map showing the <br />location of the discharges and copies of all analytical information on the discharges. Please sample the discharges for the parameters listed in item 26 and <br />submit those results. This requirement may be waived by the Division if suitable data on the discharge's historic quality exists. <br />AD <br />Additional Information: Please include any other information that you feel the Division should be aware of in drafting this permit. Include here any <br />flow or stream data that you would like considered. Attach pages as necessary. <br />See cover letter. <br />37. Stormwater Discharges: All active and inactive coal mines must be covered by a stormwater permit. If your facility does not already have a stormwater <br />permit, complete Appendix C -1 and submit in addition to this application. ALSO, SEE ITEM 38. <br />38. Stormwater Management Plan: This item applies to all renewal permit applicants where the facility has stormwater coverage through a combined <br />process water /stormwater permit (individual or general) or through separate process water and stormwater permits (and may apply to other facilities). In <br />these cases, the permittee shall sign the following certification statement. If a SWMP has not been prepared, see the SWMP guidance document in <br />Appendix C -2. <br />• <br />"I certify under penalty of law that a complete Stormwater Management Plan, in conformance with Appendix C -2 of this application, has been prepared for my facility. Based <br />on my inquiry of the person or persons who manage the system, or those persons directly responsible for gathering the information, the SWMP is, to the best of my knowledge <br />and belief, true, accurate and complete.
