24. Ditches and Storm Sewers: Is any receiving water indicated in item 22 a ditch or storm sewer? No See Item 40. <br />25. Certified Operator: Do you have a certified operator? Yes If Yes, please list name(s), certification number(s) and level(s). For information <br />regarding required certification, contact the Colorado Plant Operators Certification Board at (303) 692 -3500. <br />26. Discharge Quality: Analytical data for the parameters specified in the table beginning below and continuing on the next page, unless waived by the <br />Division, shall be submitted for at least one grab sampling of each discharge point which is not solely made up of surface runoff or domestic wastewater <br />as well as state waters upstream of each discharge. Upstream data should be from non - runoff periods, to the extent possible. Instream sampling is not <br />required if upstream flow is intermittent or representative instream data exists on file. For new mines, please submit a minimum of one year data for <br />those parameters listed below. Such data must have been obtained on at least a quarterly basis and must be reflective of the water quality prior to any <br />mining activity. See instructions.
