• <br />7 . <br />• <br />6. Industrial Activity: Describe the primary industrial activities which take place on site. Include the type of facility (underground mine, surface <br />mine, etc.) plus a brief description of the nature of the business and the industrial processes used. Include a description of the mining and coal <br />preparation processes where applicable. (The applicant may want to submit a process flow sheet.) <br />Surface coal mine reclamation, no more mining. <br />Indicate whether an exploratory, active mining, inactive mining, or post mining operation or is the site in temporary cessation. <br />Post mining at all outfalls. <br />If exploratory, please submit any known water quality /quantity data relative to the discharge(s) and the receiving stream(s) that reflect the <br />conditions prior to the present activity, the length of time the activity is expected to be under exploration and describe what activities will take <br />place during exploration that could have an impact on the quantity or the quality of the discharge. If post mining, describe areas affected and <br />extent of reclamation completed and length of time elapsed since revegetation occurred. Also, specify whether any outfalls previously described <br />serve, partially or totally, a reclaimed area. Include expected bond release date. <br />See cover letter <br />8. Production: List the principal product(s) produced and current and maximum production rates. <br />No more coal production. <br />9. Is this a seasonal operation? No If Yes, please list the months of operation. <br />10. Intermittent Discharges: Except for storm runoff, are any of the discharges described in item 21 intermittent or seasonal? <br />Yes If Yes, describe the frequency, duration and flow rate of each discharge occurrence. <br />See cover letter. <br />1 1. Location Map: A 8 1/2 x 11 inch location map designating the facility property, intake points, discharge points, each of its hazardous waste <br />treatment storage or disposal facilities, areas in post mining, each well where fluids from the facility are injected underground, those wells, springs, <br />other surface water bodies and drinking water wells listed in public records or otherwise known to the applicant and the receiving waters shall be <br />submitted. The map shall extend one mile beyond the property boundaries. The map shall be from a 72 or 15 minute USGS quad sheet, or a map <br />of comparable scale. A north arrow shall be shown. A larger map may be included for reference if the permittee wishes. <br />THE MAP MUST BE ON PAPER 8 1/2 X 11 INCHES OR PROCESSING OF YOUR PERMIT WILL BE DELAYED . <br />12. Site Map: A 8 1/2 x 11 inch map of the site shall be submitted showing facility features (whenever possible) such as buildings, ponds, diversion <br />ditches, stream locations, numbered discharge points, sampling and flow monitoring points, land application areas, stockpiles, waste rock piles, <br />topsoil piles, location of french drains and /or spoils springs, crusher areas, mine drainage flow paths, domestic wastewater treatment plants, power <br />plants, truck washing areas, explosive storage areas, parking lots, vehicle maintenance areas, chemical storage areas, etc. The outfalls shall be <br />labeled to correspond with the numbers listed in item 21. A larger map may be included for reference if the permittee wishes. <br />THE MAP MUST BE ON PAPER 8 1/2 X 11 INCHES OR PROCESSING OF YOUR PERMIT WILL BE DELAYED . <br />13. Water Balance: Attach a line drawing showing all water flow through and from the mine /preparation plant sites. Indicate sources of intake water, <br />operations contributing wastewater to the effluent, and treatment units labeled to correspond to the more detailed descriptions in item 21. <br />Construct a water balance on the line drawing by showing average flows between intakes, operations, treatment units and outfalls. If a water <br />balance cannot be determined, provide a pictorial description of the nature and amount of any sources of water and any collection or treatment <br />measures.
