The insurance certificate covers the time period from October 1, 2011 <br />through September 30, 2012. The certificates list the insured as Energy Fuels Corporation. The <br />certificate dated October 4, 2010, also lists the description of the operations as the "Southfield Mine - <br />Permit #C -81- 014 ". The Division accepts the Certificate of Liability Insurance received October 4, <br />2011. <br />Please assure that a copy of this updated certificate of insurance is filed at the mine office with the <br />public records.
