September 30, 2011 C- 1981- 008/New Horizon Mine MLT <br />Annual Reclamation Reports, Annual Hydrology Reports, and NPDES Permit and Records, <br />have been moved to the Training Room. Pre -Blast Surveys and older Blasting Records are <br />kept in the mine's Storage Room, located in a third building. <br />• Records for public viewing are deposited at the Montrose County Courthouse Nucla Annex. <br />Available records included a full set of the permit volumes; the TR -60 application; some TR- <br />57 and MR -78 materials; and an SL -12 binder. <br />BACKFILL and GRADING — Rule 4.14 <br />Contemporaneous Reclamation 4.14.1; Approximate Original Contour 4.14.2; Highwall <br />Elimination 4.14.1(2)(f); Steep Slopes 4.14.2, 4.27; Handling of Acid and Toxic Materials <br />4.14.3; Stabilization of Rills and Gullies 4.14.6: <br />• The backfilling sequence behind the currently active pit was clearly visible (Photo 1): lower <br />portions of the pit are backfilled with `Bench 2" material that has been blasted and either <br />pushed by dozer or hauled and dumped by truck. Upper portions of the backfill consist of <br />"Bench 1" material that has been free -dug by the shovel, hauled around the pit, by truck, <br />dumped, and graded. <br />• Western Fuels is carefully monitoring the amount of Bench 1 available for backfilling on the <br />Morgan and WFC parcels, to ensure that approved thicknesses and approximate original <br />contour will be achieved. <br />• The backfilled slope located immediately west of Pond 012 has been recently graded smooth, <br />with furrows following the contour to prevent erosion. <br />EXPLOSIVES — Rule 4.08 <br />Distance Prohibitions 4.08.4; Warnings 4.08.4; Control of Adverse Effects 4.08.4: <br />• Mr. Fry said that a Pre -Blast Survey was conducted for a new dwelling located on the <br />Enstrom (Nancy) property, west of the original house, and further from the permit boundary. <br />• The Division has not yet been provided with a copy of this survey. Please submit a copy of <br />the Enstrom Pre -Blast Survey, in accordance with the requirements of Rule 4.08.2(1) and <br />(3). <br />HYDROLOGIC BALANCE - Rule 4.05 <br />Drainage Control 4.05.1, 4.05.2, 4.05.3; Siltation Structures 4.05.5, 4.05.6; Discharge Structures <br />4.05.7, 4.05.10; Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water Monitoring 4.05.13; <br />Surface Water Monitoring 4.05.13; Drainage — Acid and Toxic Materials 4.05.8; hnpoundments <br />4.05.6, 4.05.9; Stream Buffer Zones 4.05.18: <br />• The CCC Ditch continues to flow. Mr. Fry said that it is rumored that the ditch will be <br />turned off on October 8th.
