The plan is incomplete; therefore, I require that you provide <br />the following information at your earliest convenience: <br />1. Section 3.4 — Ventilation/Escape Raise Rehabilitation: The plan amendment describes the <br />location from FS 505, and references Figure 1. Site and Layout Plan. Provide accurate <br />location of the proposed activity. This feature was not surveyed during the onsite visit, <br />and an additional onsite visit will be required. <br />Provide a plan view of the new portal area and associated road, along with proposed <br />storm water diversions, if any (Example, above the face up area, and likely areas where <br />sediment control such as wattles, etc. will be used). Additional portal engineering <br />designs if available. <br />3. Provide an estimation of the loose cubic yardage that will be generated from the decline <br />construction and estimate the volume to be used for portal pad construction and the <br />volume to be stored on private surface. <br />4. Section 6.1 — Portal Area Facilities: Provide a location on the pad layout design where <br />the facilities on the portal pad will be stored and utilized. (Example (2) 10,000 gallon <br />water tanks, electric substation, parking, cargo boxes, etc.) <br />5. Section 11 -3: Provide a brief narrative discussing the current Storm Water Management <br />Plan requirements that were set forth several years ago for the existing reclamation area. <br />6.
