OBSERVATIONS <br />PERMIT #: M -1999 -003 <br />INSPECTOR'S INITIALS: CBM <br />INSPECTION DATE: August 16, 2011 <br />1) The Division conducted a monitoring inspection of the site on 8/16/11. The site was not active at the time of the <br />inspection. <br />2) The site is a small 110 sand and gravel operation located approximately 14 miles north of Parkdale Colorado. <br />The operator was contacted but was unable to attend. <br />3) The active mining pit is along the south side of the permit. The pit slopes appeared stable .There was a white <br />pickup truck parked along the north side of the active pit area. There were also various size proceed material <br />stockpiles noted along the west side of the active pit area. Along the East side of the main pit area, there was <br />what appeared to be overburden/ topsoil stockpiled. The stockpile was stable and vegetated. <br />4) Even though no drainage channel leading from the pit area to the access road was noted at the time of the <br />inspection, there is a good chance that some affected drainage can leave off site and reach Tallahassee road <br />drainage. The operator is encouraged to place a sediment control structure to limit drainage from the access <br />road from reaching Tallahassee road. <br />5) A permit sign and boundary markers were properly posted. The current warranty for the site in the amount of <br />$34,685.00 is considered adequate. <br />Page 2 of 4 <br />
