PERMIT #: M- 2002 -119 <br />INSPECTOR'S INITIALS: DMC <br />INSPECTION DATE: September 28, 2011 <br />OBSERVATIONS <br />This inspection was conducted by the Division of Reclamation, Mining and Safety (Division) in <br />response to the Operator's request to release the 1.8 acre staging area from further reclamation <br />responsibility. Gayle Lyman, representing the Operator, was present during this inspection. <br />Photographs are included at the end of this report in order to illustrate some of the conditions <br />observed during this inspection. <br />The Fetcher/Vale Pit is located approximately 20 miles north of Steamboat Springs and accessed <br />from CR62. The site is a 112c operation that includes a total of 25.3 permitted acres. The <br />Division currently holds a financial warranty amount of $171,100.00 for this site. The Division <br />will review the reclamation costs for this site in order to ensure the financial warranty reflects the <br />actual current cost of fulfilling the requirements of the Reclamation Plan. The Operator will be <br />notified if the Division determines that the current financial warranty amount is insufficient. <br />Adequate mine identification signage was noted at the entrance to the site in accordance with <br />Rule 3.1.12(1). <br />The affected area was marked by markers that were clearly visible and adequate to delineate the <br />affected area boundary in accordance with Rule 3.1.12(2). <br />The Operator has requested that the 1.8 acre staging area be released from the reclamation <br />permit. Pursuant to the approved reclamation plan for this site, the post mining use for the <br />staging area is to be for agricultural storage. The area is to be graded and graveled, but not <br />revegetated. The fuel tank and and lined containment berm for the tank are to be removed. It <br />was noted during this inspection that the operator has reclaimed the staging area in accordance <br />with the approved reclamation plan. <br />The fuel tank formerly located in the staging area had been placed in a bermed, lined <br />containment structure located in the northeast portion of the pit. <br />Mining was occurring at the site during this inspection, in the west portion of the pit. <br />No problems or violations were noted during this inspection. <br />Responses to this inspection report should be directed to Dustin Czapla at the Division of <br />Reclamation, Mining and Safety, Grand Junction Field Office, 101 South 3rd Street, Room 301, <br />Grand Junction, Colorado 81501, phone number (970) 243 -6299. <br />Page 2 of 3 <br />
