RE: Roadside Portals (Permit No. C -1981 -041)
Technical Revision No. 62 (TR -62) Adequacy Review
The Division of Reclamation Mining, and Safety (Division) has reviewed the above referenced
technical revision application submitted to change the permit boundary and update associated
acreages resulting from the recent Phase III bond releases SL -5, 6, and 7. SCC also requested
specific hydrologic monitoring plan and reclamation plan changes and updated miscellaneous
ownership and control issues. We have reviewed the application in its entirety and have the
following questions and comments.
Ownership and Control Information
Rule 2.03.4(3) requires the applicant to provide the name, address, telephone number,
social security number and employer identification number of each person who owns or
controls the applicant. Please provide the telephone number for AEP Coal, Inc. (listed on
revised page 3 -1).
2. The Division has reviewed the ownership and control section of the currently approved
permit application package for the Roadside Portals, comparing the section with the
current information in the Applicant Violator System (AVS). There are numerous
discrepancies that will need to be addressed by Snowcap Coal Company prior to approval
of the revision.
a. Snowcap Coal Company
Proposed page A3 -1 -1 includes new officers James D. Henry, Mark C. McCullough, and
Jon D. Holtzmuller. These individuals do not appear to be currently listed in AVS and do
not have an AVS entity number. Please either provide the AVS entity number of these
individuals, or provide social security numbers for each of them so that the Division can
enter them as new entities in AVS.
