Preliminary review of the Application determined that the following items <br />must be received before the Division can consider your application as being submitted and <br />technical review can begin: <br />Application Form <br />1. The 110 Construction Materials Reclamation Permit Application package which was <br />submitted only contained the first page. Please fill out and submit the entire Application form <br />(copy enclosed). <br />2. The Applicant has listed the permitted acreage as 8.4 acres on the first page of the <br />Application form. The Division's records show the Chartier Pit is permitted for 8.5 acres. <br />The maximum acreage which can be permitted under a 110 Construction Materials <br />Reclamation Permit is 9.9 acres. Therefore, the Applicant may amend the current permit by a <br />total of 1.4 acres. Please revise Item No. 3 of the Application form accordingly. <br />3. The Applicant has not provided the Operator or Company name to be used on the permit. <br />Please provide the Operator or Company name to be used on Item No. 1 of the Application <br />form. If the Operator's name is to be used on the permit, then list the `Type of Organization' <br />under Item No. 1.1 as Individual. <br />Amendment to a Permit (Rule 1.10) <br />4.
