Compliance With Mine Plan <br />NA - Support Facilities Not On -site <br />R - Other <br />NA - Special Categories Of Mining <br />NA - Processing Waste <br />Y - Topsoil <br />INTRODUCTION: <br />COMMENTS <br />This was a complete inspection of the Fruita Mine conducted by Mike Boulay of the Division of <br />Reclamation, Mining and Safety Grand Junction Field Office on August 30, 2011. The mine permit was <br />revoked and the bond forfeited in 1994. The site is fully reclaimed. <br />There has been some activity at the site since the last inspection conducted in June. Since the last <br />inspection, the lower reclaimed road segment that heads northeast from the sediment pond has been <br />blocked with a cable and a No Trespassing sign posted (see attached Photograph No. 1). During this <br />inspection, I also noted a new structure at the end of this road (see Photo No. 2). It appears to be an <br />excavation covered by wood planking with posts and rails. A closer view of this newly constructed <br />feature is shown in Photo No. 3. <br />AVAILABILITY OF RECORDS — Rule 5.02.4(1): <br />This is a bond forfeiture site and all applicable records are maintained by the Division. <br />BACKFILL and GRADING — Rule 4.14 <br />Contemporaneous Reclamation 4.14.1; Approximate Original Contour 4.14.2; Highwall Elimination <br />4.14.1(2)(f); Steep Slopes 4.14.2, 4.27; Handling of Acid and Toxic Materials 4.14.3; Stabilization of Rills and <br />Gullies 4.14.6: <br />The portal backfill area is shown in Photo No. 4.
