We will send you copies <br />of the approved documents for your records. For your convenience, you can scan the signature <br />pages and email them to lmauch @fs.fed.us to expedite approval. <br />I have also enclosed a bill which reflects an annual fee of $158.81 and a one time Cost Recovery <br />fee of $112.00. There are two copies of this bill.
