EXHIBIT C - Pre - mining and Mining Plan Map(s) of Affected Lands (Rule 6.4.3) <br />5. The Applicant has stated the requirements for Exhibit C have been satisfied under the <br />original permit submittal. The Applicant must submit a new Pre - Mining and Mining Plan <br />Map which depicts the revised permit boundary. As required by Rule 6.2.1(2), all maps must <br />show the name of the applicant, must be prepared and signed by a qualified person, give the <br />date prepared, and identify and outline the area which corresponds with the application. In <br />addition, the Pre - Mining and Mining Plan Map(s) must portray the following: <br />a) All adjoining surface owners of record. <br />b) The name and location of all creeks, roads, buildings, oil and gas wells and lines, and <br />power communication lines on the area of affected land and within 200 feet of all <br />boundaries of such area. <br />c) The existing topography of the area with contour lines of sufficient detail to portray the <br />direction and rate of slope of the affected land. <br />d) The total area to be involved in the operation, including the area to be mined and the area <br />of affected lands. <br />e) The type of present vegetation covering the affected lands. <br />f) Show the owner's name, type of structures, and location of all significant, valuable and <br />permanent man-made structures within 200 feet of the affect land. <br />EXHIBIT F - Reclamation Plan Map (Rule 6.4.6) <br />6. The Applicant must submit a new Reclamation Plan Map which depicts the revised permit <br />boundary. The map must portray the expected physical appearance of the area of the affected <br />land, correlated to the proposed mining and reclamation timetables. The map must show <br />proposed topography of the area with contour lines of sufficient detail to portray the direction <br />and rate of slope of all reclaimed lands. <br />EXHIBIT N - Source of Legal Right to Enter (Rule 6.4.14) <br />7. The Applicant must demonstrate a legal right to enter the amended property. This may be a <br />copy of an access lease, deed, abstract of title, or a current tax receipt. A signed statement by <br />the Landowner and acknowledged by a Notary Public stating that the Operator /Applicant has <br />legal right to enter and mine is also acceptable. <br />EXHIBIT R - Proof of Filing with County Clerk and Recorder (Rule 6.4.18) <br />8. The Applicant must submit proof of filing with the County Clerk and Recorder. Proof of filing <br />may be in the form of an affidavit or receipt indicating the date on which the application was <br />placed with the County Clerk and Recorder. If the County Clerk and Recorder will not provide <br />you with a receipt, then you may make your own receipt to be signed or date stamped by the <br />County Clerk and Recorder. <br />
