G' e. t v- <br />1 <br />"TM -1 <br />R ecovvio <br />i g Q11 <br />Di Wi <br />Wing d Safety <br />American Shale Oil, LLC (AMSO) is submitting the attached information in accordance with Appendix B <br />(Logging and Testing Requirements) of AMSO's Underground Injection Control (UIC) Program Permit <br />(Permit No. CO52154- 08524): <br />A step rate test was performed to provide the information needed to confirm that the Maximum Allowable <br />Injection Pressure (MAIP) established in the UIC permit does not exceed the fracture pressure of the <br />injection zone. The MAIP specified in the permit is 1,025 psi. The test was conducted in accordance with <br />AMSO's revised step rate testing procedure that was approved by EPA on April 4, 2011. Testing was <br />performed on May 3, 2011 by Single Jack Testing & Services, Inc., under the supervision of AMSO's on- <br />site geological consultant. A cross section showing the well and test configuration, a step rate test report, <br />and the EPA - approved testing procedure are presented in Attachment A.
