Box 667 <br />Craig, CO 81626 <br />OBSERVATIONS <br />PERMIT #: M- 2011 -036 <br />INSPECTOR'S INITIALS: DMC <br />INSPECTION DATE: August 4, 2011 <br />This pre- operation inspection was conducted by the Colorado Division of Reclamation, Mining and Safety in <br />order to document the pre- mining conditions at the site and to verify the accuracy of the materials submitted <br />with the 112c Reclamation Permit application. Marvin Moore, representing Moffat County, was present during <br />this inspection. Photographs are included at the end of this report in order to document some of the conditions <br />observed during this inspection. <br />The proposed operation is located approximately 20 miles west of Maybell and access from CR10. The site <br />will include a total of 86.4 permitted acres. <br />In accordance with Rule 4.1.2(2), a Financial Warranty will not be required for this site. <br />Adequate notice was posted at the site in accordance with Rule 1.6.2(b). <br />The permit boundary was adequately marked with stakes and survey lathe in accordance with Rule 3.1.12(2). <br />The primary commodity to be mined at this site will be the limestone outcropping in an east -west trending ridge <br />in the central part of the permit area. Secondary commodities may include clay, and /or sand and gravel. <br />Vegetation observed at the site was typical of sagebrush communities in the region. Some species observed at <br />the site included sage brush, Indian ricegrass, needle and thread, wheat grass and rubber rabbitbrush. <br />Pre - existing significant, valuable or permanent man -made structures located on or within 200 feet of the <br />affected area that were not noted in the permit application, were not noted during this inspection. <br />Noxious weeds were not observed at the site during this inspection. <br />No problems or violations were noted during this inspection. <br />Responses to this inspection report should be directed to Dustin Czapla at the Division of Reclamation, Mining <br />and Safety, Grand Junction Field Office, 101 South 3rd Street, Room 301, Grand Junction, Colorado 81501, <br />phone number (970) 243 -6299. <br />Page 2 of 2 <br />
