PROBLEM /POSSIBLE VIOLATION: Problem: The site is utilizing off -site fill that is not consistent with the
import fill Notice submitted by the Operator on December 30, 2008.
CORRECTIVE ACTIONS: The Division will further investigate this issue in order to determine the
necessary corrective actions.

3. INSPECTION TOPIC: Hydrologic Balance
PROBLEM /POSSIBLE VIOLATION: Problem: The site is utilizing off -site fill that is not consistent with the
import fill Notice submitted by the Operator on December 30, 2008. The imported fill has potentially
negative impacts to the groundwater system.
CORRECTIVE ACTIONS: The Division will further investigate this issue in order to determine the
necessary corrective actions.
