Hickenlooper <br />Governor <br />Mike King <br />Executive Director <br />Loretta Pineda <br />Director <br />Re: Corkle Construction Pit #2, File No. M011 -033, Receipt of 110 Construction Materials <br />Reclamation Permit Application Package — Third Incompleteness Letter <br />On June 13, 2011 the Division of Reclamation, Mining and Safety (Division or DRMS) received the <br />110 Construction Materials Limited Impact Operation Reclamation Permit Application package for <br />the Corkle Construction Pit #2, File No. M- 2011 -033. Review of the supplemental information <br />received on August 4, 2011, determined the following items must be corrected before the <br />Division can consider the application submitted and technical review can begin: <br />Application Form: <br />1. The Division requires the Applicant /Operator name remain consistent and matches the exact <br />true name or trade name registered with the Secretary of State's office ( "Harry Edward <br />Corkle" or "Harry Corkle Const ") throughout the application. The Applicant did not revise the <br />Applicant /Operator section on page 7 correctly. Please provide the Division with a revised <br />and original (signatures in blue ink) copy of the application form. A copy of a re- edited <br />Certification Page of the application form is attached for reference. <br />2. The Notary Public section on the Certification Page (page 7) of the application form was not <br />completed properly. Please submit a revised Certification Page to the Division with the <br />required corrections. A copy of an edited Certification Page of the application form is <br />attached for reference. <br />Exhibit I - Proof of Filing with County Clerk (Rule 6.3.9): <br />3. Please provided an affidavit or receipt indicating the date on which the revised application <br />information in response to this letter was placed with the local County Clerk and Recorder <br />for public review, pursuant to Subparagraph 1.6.2(1)(c). <br />S , Office of <br />Denver • Grand Junction • Durango - Active and Inactive Mines <br />
