The Division has the following comments regarding <br />ownership and control information required by Rule 2.03.4 as compared to the national Applicant <br />Violator System (AVS): <br />1. The Applicant Violator System (AVS) lists Chuck Hagel as a Director of Chevron Mining, Inc. <br />The information submitted by Chevron does not include this individual in the list of officers for <br />Chevron Mining, Inc. Please either update the permit application with the correct information or <br />provide information to the AVS administrator to disassociate this individual with Chevron <br />Mining Incorporated. <br />2. The information submitted by Chevron Mining, Inc. for parent corporation Chevron Global <br />Energy, Inc. did not match the information currently displayed in AVS. The following entities <br />are listed in AVS differently than in the information submitted by Chevron. Please either update <br />the permit application with the correct information or provide information to the AVS <br />administrator indicating end dates for the following individuals: <br />a. Patrick A. Wright is listed in AVS as an Assistant Treasurer; there is no mention of this <br />individual in the information provided by Chevron. <br />b. Michael K Wirth is listed in AVS as a Vice President; there is no mention of this <br />individual in the information provided by Chevron. <br />c. Dale A Walsh is listed in AVS as a Director and Vice President, although Chevron <br />reports that this individual left office on April 1, 2010. <br />d. Gary D Yesavage is listed in AVS as a Director and Vice President, although Chevron <br />reports that this individual left office on May 1, 2009. <br />e.
