The following information will be recorded <br />using water proof ink: <br />• Names of personnel <br />• Associated field form number for each location sampled. <br />• Weather conditions <br />• Date and time of sampling <br />• Location and well number <br />• Parameter stabilization info. (time, values, etc.). <br />• Condition of the well <br />• Decontamination information <br />• Calculations (e.g., calculation of evacuated volume) <br />• Calibration information <br />• Sampler's signature <br />o Other pertinent information (problems, special conditions, etc.) <br />0 -12 <br />
