New Access Road Construction Work Plan <br />Exhibit D — Attachment 1 <br />• Wildcat will retain construction contractor licensed in the state of Colorado with <br />experience in successfully constructing hillside roads; <br />• Wildcat will hold a project kick -off meeting to clarify project objectives, roles and <br />responsibilities of participants, project schedule, safety requirements submittal and <br />reporting requirements. The kick -off meeting will be attended by representative(s) of <br />Wildcat, including the geotechnical engineer and Technician, contractor's project <br />manager, superintendent, and health and safety officer, and regulatory authority <br />representatives; and <br />• Utilizing engineering drawings and topographic features (Attachment D -1- Sheets 1 <br />thru 21); <br />• Delineate permit boundaries with permit boundary markers with flags and or <br />stakes; <br />• Establish Profile and Cross Section Stakes for construction purposes. (See <br />Attachment D -1); <br />• Establish cut and fill stakes per the engineering drawings (See Attachment D -1- <br />CLC Associates); <br />• Delineate areas for temporary storage of debris, and wood. The boundary of the <br />storage area will be marked with stakes and or flagging tape. <br />Construction Summary and Scheduling <br />• Establish health and safety procedures; <br />• Establish site management communication protocols; <br />• Establish site maintenance requirements; <br />• Establish grade stakes in accordance with approved design drawings; <br />• Remove materials from inside road cuts using a dozer; <br />• Install pre - construction BMPs: <br />• Catch basins Exhibit D (Attachment D -1- Sheet C4.92) /rock check dams <br />(backhoe); <br />• Inside Road Ditches (backhoe /dozer); <br />• Wattles (erosion logs) /Rock barriers (manual labor); (Attachment D -1- Sheet <br />C4.90); <br />• Cross Drains (backhoe /dozer); <br />• Ditch check dams (manual labor); and, <br />• Wetland barriers (manual labor). <br />• Grade the existing road surface to drain at least 2% to the inside road cut (Attachment <br />D -1 - Sheet C2.00); <br />• Remove material from the cut slope to desired slope and road width; <br />• Removed terrace rock will be screened and crushed as necessary using portable <br />equipment; <br />Wildcat Mining Corporation <br />May Day Idaho Mine Complex — 112d Permit Application <br />Revised August, 2011 <br />Exhibit D — Attachment 1 <br />Page 6 of 15 <br />
