PERMIT #: M -1994 -011 <br />INSPECTOR'S INITIALS: GRM <br />INSPECTION DATE: July 19, 2011 <br />No mine related debris or trash was observed. There is some car body parts scattered on the upper bench. <br />The operator notes that the debris is not related to any of the activities associated with the pit. The inspector <br />notes this only for reference during any future reclamation activities. <br />No fuel or petroleum products were noted on site. Based on recent heavy precipitation in the area <br />stormwater controls, erosion, and sedimentation do not appear to be an issue at this time. <br />The financial warranty has not been reviewed since the conversion in 2002. The inspector will update the <br />bond based on current conditions and forward to the operator by separate cover. Please note that due to the <br />length of time between reviews substantial increases in the price of seed and fuel will impact the estimated <br />liability. <br />The landowner contacted the Division in regard to the status of this permit and reclamation plans. Several <br />issues were brought up, including the lack of progress toward reclamation and site access. The landowner <br />brought up that the lease had expired.
