CO 80308 <br />RE: Deepe Farm Pit, Technical Revision Approval, Permit No. M -81 -302, Revision No. TR-6 <br />Dear Mr. Plummer: <br />On September 15. 1997 the Division of Minerals and Geology approved the Technical Revision application submitted to <br />the Division on May 02, 1997, addressing the following: <br />Modify the side slopes and final elevations of a portion of the berm along the southeast side of the Deepe Farm Pit. <br />The terms of the Technical Revision No. 6 approved by the Division are hereby incorporated into Permit No. M -81 -302. <br />The conditions of the Division approval are noted below. Please note that all other provisions of Permit No. M -81 -302 <br />remain in full force and effect. <br />Stipulation No. Description <br />5 -1 The operator will submit a construction report, including documentation of any test results, within 60 <br />day of the completion of the earthwork on the Deepe Farm Pit perimeter levee. The report shall detail <br />how the specifications approved in technical revision TR -06 were achieved. The operator will also <br />provide a certification from the construction quality assurance engineer that the levee modifications were <br />completed in accordance with the design approved in the technical revision. <br />5 -2 All areas on the out slopes of the levee must be protected by the establishment of vegetation or the <br />placement of riprap.
