Days <br />S <br />M <br />T <br />W <br />T <br />F <br />S <br />Work Shifts <br />- <br />D <br />D <br />D <br />D <br />D <br />D <br />D = Day Shift N = Night Shift <br />ame <br />VLF EARTHWORKS FIELD MONITORING SUMMARY (WEEKLY REPORT) <br />Project Title: CC &V/ Phase 5 <br />Owner: Cripple Creek & Victor Gold Mining Co. <br />Location: Cripple Creek & Victor Gold Mine <br />Work Element: Phase 5 CQA/ VLF <br />Date: October 2, 2010 <br />Reporting Period: 9/26/10 through 10/02/10 <br />Project No.: 84201125E0 <br />Contractor: Ames Construction Co., Inc. <br />Shift: Day/Night <br />Ambient Temperature Range (F): Low: 40 °F to 51°F ( + / -) High: 65 °F to 80 °F ( + / -) <br />AMEC CQA: Personnel On -site through the Reporting Period (See Attachment A). <br />AMES: No issues. <br />CONSTRUCTION ACTIVITIES and PROGRESS: <br />I) EARTHWORKS <br />A) Subgrade Preparation <br />1) VLF <br />Subgrade was inspected approved for soil liner fill placement by an AMEC field representative <br />in the area shown on Figure 1. <br />B) Slope Contouring <br />1) VLF <br />Slope contouring was performed between stations A51 +00 ( + / -) and A62 +00 ( + / -), <br />approximately 0 to 100 feet right and left of centerline, between stations J9 +00 ( + / -) and <br />J16 +00 ( + / -), approximately 0 to 200 feet left of centerline and between stations K4 +00 ( + / -) <br />and K7 +00 ( + / -), approximately 50 to 150 feet left of centerline using various CAT dozers and <br />one Komatsu 300 excavator. <br />C) Cut to Fill <br />1) VLF <br />A CAT 385 excavator was used to excavate material from a designated area in the Cresson pit. <br />The material was loaded into CAT 777 and 40 -ton articulating haul trucks and hauled between <br />stations A60 +00 ( + / -) and A62 +00 ( + / -), approximately 20 feet left of centerline to 20 feet <br />right of centerline. Structural fill was placed in 10" to 24" lifts using various CAT dozes. Each <br />lift was moisture conditioned using a CAT 773 water truck (when necessary) and compacted <br />VLF Weekly Report WE 10.02.10.D <br />
