PERMIT #: M- 1982 -200 <br />INSPECTOR'S INITIALS: BMK <br />INSPECTION DATE: June 17, 2011 <br />OBSERVATIONS <br />1) The Division conducted a joint monitoring inspection of the site with Stephanie Carter of BLM. The site is located <br />approximately 15 miles southwest of Cripple Creek. The site was not active at the time of the inspection. <br />2) The small granite quarry covers a total of four acres. According to the annual report is only used on as need basis's. A <br />permit sign and boundary corners were properly displayed at the time of the inspection. The entire permit area was walked <br />and inspected and no problem noted. The pit high walls were stable. A small water from the recent rain was noted <br />impounded along the south east side of the active pit area. <br />3) No noxious weed infestation was noted at the time of the inspection. The current financial warranty in the amount of <br />$5,00.00 is deemed adequate to cover the cost of reclamation. The operation appeared to be in compliance with the <br />approved reclamation plan. <br />' Page 2 of 4 <br />
