These documents include information regarding disturbance <br />that has occurred to date, disturbance envisioned for the next reporting period and <br />reclamation accomplished to date and planned for the next reporting period. Also included <br />is an updated map showing the current status of the well field and the signed certification <br />form. A check for $1,150 to cover the annual fees was sent to the Denver office earlier.
