Compliance With Mine Plan <br />NA - Support Facilities Not On -site <br />R - Other <br />NA - Special Categories Of Mining <br />NA - Processing Waste <br />Y - Topsoil <br />COMMENTS <br />INTRODUCTION: <br />This was a complete inspection of the Fruita Mine conducted by Mike Boulay of the Division of Reclamation, <br />Mining and Safety Grand Junction Field Office on June 29, 2011. The mine permit was revoked and the bond <br />forfeited in 1994. The site is fully reclaimed and there is no activity at the mine site. <br />AVAILABILITY OF RECORDS — Rule 5.02.4(1): <br />This is a bond forfeiture site and all applicable records are maintained by the Division. <br />BACKFILL and GRADING — Rule 4.14 <br />Contemporaneous Reclamation 4.14.1; Approximate Original Contour 4.14.2; Highwall Elimination <br />4.14.1(2)(f); Steep Slopes 4.14.2, 4.27; Handling of Acid and Toxic Materials 4.143; Stabilization of Rills and <br />Gullies 4.14.6: <br />The portal backfill area is shown in Photo No. 1. There are no slumps or cracks present at the 41 portal backfill <br />area. The site remains stable with no significant erosion or instability problems identified during this inspection. <br />HYDROLOGIC BALANCE - Rule 4.05 <br />Drainage Control 4.05.1, 4.05.2, 4.053; Siltation Structures 4.05.5, 4.05.6; Discharge Structures 4.05.7, <br />4.05.10; Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water Monitoring 4.05.13; Surface Water Monitoring <br />4.05.13; Drainage — Acid and Toxic Materials 4.05.8; Impoundments 4.05.6, 4.05.9; Stream Buffer Zones 4.05.18: <br />The mine site was very dry at the time of the inspection. The sediment pond was completely dry. The pond <br />embankment and spillway are stable. The permanent drainage channel appears stable and in good condition.
