Please find a copy of the approved well permit. (EXHIBIT 1) <br />3. The following are the specifications for the well pump. <br />a. Well pump will not exceed 200 gallons per minute <br />b. 20 HP 460 Volt 3 Phase Motor <br />c. 200 GPM 20HP 6" B Series Turbine Submersible Pump End <br />June 21, 2011 <br />Re: 1990 (CON010) <br />COC74961 <br />Your Letter 6/14/11 <br />1. The access road is as it currently exists. No changes will be made to the current road. <br />This road is not considered a haul road, and will have minimal usage. One to two <br />vehicles per day. This road will be water sprayed for dust containment. <br />4. The plugging and abandonment plan for the well as attachment. Also please see Exhibit <br />G of the original permit application. (EXHIBIT 2) <br />5. Please see attached design drawings of the tailings and water storage pond. (EXHIBIT 3) <br />6. Please see attached conceptual cross section of the mining area.
