(GCC) hereby submits the enclosed application to amend the referenced Regular <br />112 Operation Permit. This amendment modifies the Reclamation Plan (Exhibit E) and Reclamation <br />Costs (Exhibit L) based on a recent clarification of the Pueblo County Special Use Permit for this facility. <br />A copy of the Memorandum, dated June 8, 2011, is enclosed. <br />Additionally, this amendment updates the cement plant site plan and transmits copies of as -built <br />drawings. The addition and modification of structures has not resulted in any changes to the Permit <br />Boundary. <br />Enclosed are the following: <br />1. GCC's check in the amount $2,229 to cover the permit amendment fee. <br />2. One original and four copies of: <br />a. The completed and notarized Construction Material Regular (112) Operation Permit <br />Package (original with signatures in blue ink). <br />b. Amended Exhibits E and L, new Exhibit C -3 -1 (Cement Plant Site Plan Addendum) and <br />as -built drawings. <br />c. Proof of delivery of Notice of Filing Application to the Pueblo County Board of County <br />Commissioners and the South Pueblo County Conservation District. <br />d. Addendum 1 — Notice Requirements to comply with the provisions of Rule 1.6.2(1)(b) for <br />posting a notice at the site location. <br />Please contact me if you have any questions about this permit application.
