PERMIT #: M- 1981 -235 <br />INSPECTOR'S INITIALS: DMC <br />INSPECTION DATE: June 17, 2011 <br />OBSERVATIONS <br />This inspection was conducted as part of the Division of Reclamation, Mining and Safety's (Division) normal <br />monitoring program. Chris Roeder, representing the Operator, was present during this inspection. <br />Photographs are included at the end of this report in order to illustrate some of the conditions observed <br />during this inspection. <br />The West Tennessee site is located approximately 7 miles northwest of Leadville and accessed from CR 19- <br />approximately 2 miles up CR 19 from Hwy 24. The site is a 112c sand and gravel operation that includes a total <br />of 139 permitted. The Division currently holds a financial warranty amount of $132,400.00 for this site. The <br />Division will review the reclamation costs for this site in order to ensure the financial warranty reflects the <br />actual current cost of fulfilling the requirements of the Reclamation Plan. The Operator will be notified if the <br />Division determines that the current financial warranty amount is insufficient. <br />Adequate mine identification signage was noted at the entrance to the site in accordance with Rule 3.1.12(1). <br />The permit boundary was adequately marked with t -posts in accordance with Rule 3.1.12(2). Most of the <br />boundary markers are in the undisturbed forested areas surrounding the disturbed area and were somewhat <br />difficult to locate. But they were in place, and mining operations at the site have occurred within the permit <br />boundary. <br />A sediment detention pond and a wash pond are located in the northeast part of the permit area. Stockpiled <br />topsoil material imported from the Homestead Trout Club was stockpiled in this area. <br />The active pit is located in the northwest part of the permit area. The pit is approximately 35 feet deep with <br />approximately 2,500 linear feet of 1H:1V highwall. The Reclamation Plan calls for final slopes graded to 4H:1V. <br />Various stockpiles of sand and gravel are located throughout the disturbed area. <br />Two 1,000 gallon fuel (diesel) storage tanks were noted onsite that were placed in a lined and bermed area <br />that appeared to provide adequate secondary containment. Another larger fuel storage tank (approx. 20,000 <br />gallon) was located just north of the smaller tanks. The larger tank was not in use and was marked as being <br />empty. <br />The oil drums and much of the old mining equipment and debris noted in a DRMS 2004 inspection report have <br />been removed from the site.
