Please be advised of the following: <br />1. Owners name: Leadville Mining & Milling Corporation <br />700 Carr Street <br />Lakewood, Colorado 80215 <br />2. Location: A portion of Sections 28 and 33, T. 95., R. 80W., 6th P.M., <br />County of Lake, State of Colorado. <br />3. Number and Type of Holes: One (I) or two (2) observation holes for the <br />purpose of periodically measuring depth to groundwater will be <br />installed. <br />4. Estimated Depths: The estimated depth of the observation holes is 20 <br />feet. <br />Please call the undersigned if additional information is needed.
