Inspection Contact Address <br />George Robinson <br />Wildcat Mining Corporation <br />5555 DTC Parkway- Suite A -4000 <br />Greenwood Village, CO 80111 <br />PERMIT #: M- 1981 -185 <br />INSPECTOR'S INITIALS: WHE <br />INSPECTION DATE: June 2, 2011 <br />The highwall at the Chief portal is near vertical, 40+ feet high, and constructed in unconsolidated talus /soil <br />material (photo). The log canopy, originally constructed to safeguard the Chief portal, was failing due to <br />burden of sloughed material and poor support (photo). During the illegal excavation of the Chief Portal, <br />excavated materials had been placed across Little Deadwood Gulch, obstructing flow. During the time of this <br />inspection Little Deadwood Gulch was flowing, flow was visually estimated at approximately 2 cfs. Water was <br />impounded due to the obstruction associated with the construction of the Chief portal (photo). Water was <br />observed flowing from the downstream side of the inappropriately placed materials (photo). <br />Page 3 of 5 <br />
