The decision date for this permit application is August 18, 2011. <br />The Division of Reclamation, Mining, and Safety (Division) is required to make a decision by August <br />18, 2011. Therefore, your response to the following adequacy review concerns should be submitted to <br />the Division as soon as possible. <br />1. As required by Rule 1.6.5 (2), please submit the proof of publication of the public notice <br />circulated in the local newspaper and all owners of record of surface lands, and easement holders <br />that are on the affected land and within 200 feet of the boundary of the affected land, if <br />applicable. Proof of notice may be return receipts of a Certified Mailing or proof of personal <br />service. <br />Rule 6.2.1(2)(b) General Requirements of Exhibits — Maps and Exhibits <br />2. This rule requires that Maps, except the index map, must be prepared and signed by a registered <br />land surveyor, professional engineer, or other qualified person. The submitted maps do not <br />contain a signature. Please submit signed copies of these maps. <br />Rule 6.4.7 Exhibit G — Water Information <br />3. Please note and comply with the enclosed comments submitted by the Division of Water <br />Resources. <br />Rule 6.4.8 Exhibit H — Wildlife Information <br />4. As mentioned previously, the Army Corps of Engineers has indicated potential impacts to <br />Colorado Pikeminnow and razorback sucker, due to water depletions. The Division has <br />indicated the necessity to gain approval for the project from the U.S. Fish and Wildlife Service <br />(U.S.F.S.), related to these endangered species.
