Box 306 <br />Walden, Colorado 80480 <br />Re: Corkle Construction Pit #2, File No. M- 2011 -033, Receipt of 110 Construction Materials <br />Reclamation Permit Application Package — Incomplete s••' <br />Dear Mr. Corkle: <br />On June 13, 2011 the Division of Reclamation, Mining and Safety (Division or DRMS) received the <br />110 Construction Materials Limited Impact Operation Reclamation Permit Application package for <br />the Corkle Construction Pit #2, File No. M- 2011 -033. Preliminary review of the information <br />received determined the following items must be received before the Division can consider the <br />application submitted and technical review can begin: <br />Application Form: <br />1. The Division requires the Applicant /Operator name remain consistent and matches the exact <br />true name or trade name registered with the Secretary of State's office ( "Harry Edward <br />Corkle" or "Harry Corkle Const ") throughout the application. Please provide the Division a <br />revised and original (signatures in blue ink) copy of the application form. A copy of the <br />Colorado Secretary of State summary page for Harry Edward Corkle and an edited copy of the <br />application form are attached for reference. <br />2. On Item 1.1 of the Application form, the Applicant indicates the type of organization as a <br />corporation. The Applicant is listed as an individual according to the Colorado Secretary of <br />State. Please explain this discrepancy and revised the application form including the <br />Corporation Seal section on Certification Page (page 7) of the form if applicable. <br />3. On the Certification Page (page 7) of the application form the date of the Applicant's <br />signature must match the date of the notary public acknowledgement. Please submit a <br />revised Certification Page to the Division with matching dates. <br />Exhibit I - Proof of Filing with County Clerk (Rule 6.3.9): <br />4.
