PERMIT #: M-1976-051 <br />INSPECTOR'S INITIALS: DMC <br />INSPECTION DATE: May 11, 2011 <br />Another well, which was installed by the Operator in 1991 in order to increase the water supply for the <br />landowner, is located just east of the home. The ground elevation at this location is approximately 6,729 feet <br />above sea level. The water level observed in this well during this inspection was approximately four feet <br />below the ground surface. According to the completion log for this well, the stratigraphy is as follows: one <br />foot of topsoil overlies eleven feet of clay, that overlies five feet of gravel, that overlies shale. This well is <br />twenty one feet deep. This well has never been utilized. <br />The ground surface elevation was taken, using a GPS unit, from the southwest corner of the pit floor.
