O <br />a <br />Cal <br />Evergreen Analytical, Inc. <br />Date: 04-Nov-09 <br />Lab Order: 09-8452 <br />Client Project ID CASE NARRATIVE <br />SAMPLE RECEIVING <br />Sample(s) were hand delivered to the laboratory by the client. <br />Custody seals were not present. <br />The temperature of the sample(s) upon arrival was 2.10C. <br />Sample(s) were received in good condition, in the proper container, and within holding times. <br />Sample(s) were preserved properly. <br />The LIMS default sample collection time of 0000 (midnight) is listed on the reports for sample DUP- <br />001 (-04) because there is no sample collection time specified on the chain-of-custody (COC). NJO/JE <br />QUALITY ASSURANCE (QA) <br />Analyses performed on samples in this work order by EAL meet the requirements of the EAL Quality <br />Assurance Program unless otherwise explained Analyses of RCRA samples meet the requirements of <br />NELAC and Utah Rule R444-14 unless otherwise explained Analyses of discharge samples meet the <br />requirements of 40 CFR Part 136 unless otherwise explained. JE <br />CLIENT SERVICES <br />The metal, anion, and alkalinity analytes were selected per the quotation. <br />The client did not provide complete contact information on the chain of custody. <br />A modified chain of custody was faxed to the client for confirmation of analytes and reporting <br />address. <br />There are no other anomalies to report. EH <br />GENERAL CHEMISTRY <br />Method 300.0: All samples have high conductivity levels and high Chloride levels, which required <br />dilution of the samples to avoid saturating the detector. This raised the reporting limit for Nitrate-N <br />for samples MW-002 (-01), MW-003 (-02), and DUP-001 (-04) and Nitrite-N for all samples. The <br />LIMS default sample collection time of 0000 (midnight) for sample DUP-001 (-04) for nitrite and <br />nitrate, is listed as outside of holding time (H) on the reports because there is no sample collection time <br />specified on the chain-of-custody (COC). There are no other anomalies to report. BNP/MM/JE <br />METALS ANALYSIS <br />Method E200.7: The matrix spike and matrix spike duplicate (MS/MSD; on the client's sample) <br />recoveries for Calcium and Sodium are outside the QC limits due to the high concentration of Calcium <br />and Sodium in the sample versus the low concentration of the spike.
