PERMIT #: M-2005-002 <br />INSPECTOR'S INITIALS: BMK <br />INSPECTION DATE: April 11, 2011 <br />OBSERVATIONS <br />1) The site is a small sand and gravel operation located approximately 25 miles East covering 9.9 acres. Present during the <br />inspection was Mr. Dave Schuler the operator <br />2) The site has not been affected since the permit was issued in 2005, with the exception of less than 1 acre that was <br />previously affected by pre- law disturbance. The current permit area covers the previously affected acres. <br />3) The entire affected and permitted acres were walked and inspected. The only problem noted during the inspection was, the <br />lack of a permit sign at the access to the property. The lack of a permit sign is identified as a possible problem, with a <br />corrective date. Mr. Schuler stated that he will place a permit sign in accordance Rule 3.1.12(1). Adequate permit boundary <br />markers were properly posted delineating the permit area. <br />4) The current operator had not affected the site since the permit was issued in 2005.Mr Schuler plans to commence <br />operation as soon as he is able to obtain a use by special review permit from the county. <br />5) The current financial warranty in the amount of $22,500.00 is more the adequate to cover the current affected area. Since <br />the operator had never commenced mining operation on site there will be no need to be in a temporary cessation status. <br />However, the Division requests the operator to notify in writing, once the required permit is obtained from the county and <br />mining opeartion commences. <br />Page 2 of 4
