Pineda <br />Director <br />RE: Cash and Who Do Mines; DRMS File No. M0-'1983-141; Receipt of Corrective Action <br />Documentation & Request for Corrective Action Date Extension <br />Dear Mr. Beck, <br />The Division received your above referenced response in regard to the Compliance Problems <br />cited in the report of the inspection conducted on lune 12, 2009. The statuses of the corrective <br />actions regarding these problems are as follows: <br />Compliance Problem 1: The escape-way shaft has not been approved as part of the <br />reclamation plan. <br />At the request of the Operator, the corrective action date has been extended to August 31, 2011. <br />Please be aware that no extensions will be granted beyond the August 31, 2011 deadline. If the <br />Operator has not complied with the corrective action by the corrective action date, then <br />enforcement actions may be pursued. <br />Compliance Problem 2: Incorrect Financial Warranty Form <br />This problem is considered abated as a result of the Operator's submittal of a joint Financial <br />Warranty form between the Division and the Bureau of Land Management. <br />Compliance Problem 3: The permit boundary was inadequately marked. <br />This problem is considered abated as a result of the Operator's submittal of documentation <br />demonstrating that the permit boundary was surveyed and delineated. <br />The following problems were identified in the report of the inspection conducted on December <br />22, 2010. The statuses of the corrective actions regarding these problems are as follows: <br />Compliance Problem 1: An unpermitted dewatering well (CM-4) was observed in the Cash <br />Mine Area. <br />At the request of the Operator, the corrective action date has been extended to September 30, <br />2011. <br />Office of <br />Mined Land Reclamation <br />Denver • Grand Junction • Durango <br />Office of <br />Active and Inactive Mines
