Fourth Adequacy Response 112(d)-1 Application, Amended CN-01 <br />Wildcat Mining Corporation May Day Idaho Mine Complex <br />f. Type of material blasted; <br />g. Sketches of the blast pattern including number of holes, burden, spacing, and <br />delay pattern; <br />h. Sketches shall also show decking, if holes are decked to achieve different delay <br />times within a hole; <br />i. Diameter and depth of holes; <br />j. Types of explosives used; <br />k. Total weight of explosives used per hole and maximum weight of explosives used <br />per 8-millisecond period; <br />1. Initiation system; <br />m. Type and length of stemming; <br />n. Mats or other protections used; <br />o. Type of delay detonator and delay periods used; <br />p. Number of persons in the blasting crew; and <br />q. Seismographic and airblast records, where required, including: <br />(1) Type of instrument, sensitivity, and the calibration signal of the gain setting or <br />certification of annual calibration; <br />(2) Exact location of instrument and the date, time and its distance from the blast; <br />(3) Name of the person and firm taking the reading; <br />(4) Name of the person and firm analyzing the seismographic record; <br />(5) The vibration and/or airblast level recorded; and <br />r. Reasons and conditions for each unscheduled blast. <br />Response #2 <br />The Company will conduct and report blasting activities as described in Mr. Cazier's <br />summary. See Attachments G (Revised Mining Plan) and P (Revised Section 6.5) to the <br />Company's Response to CN-01 Third Adequacy Response submitted June 6, 2011. <br />Adequacy Issue #3 <br />1. In the Division's third adequacy letter, under adequacy item number 22, the Division <br />indicated additional adequacy issues regarding the proposed plan for reclamation of Little <br />Deadwood Gulch, which was adversely impacted by the illegal construction of the Chief <br />Portal, would be forthcoming. Division staff member Tim Cazier has completed his <br />review of the proposed plan and has provided a review memorandum, which is attached <br />to this correspondence. Please address the adequacy issues identified by Mr. Cazier in <br />the attached review memorandum dated May 27, 2011, "May Day & Idaho Mine <br />Complex - Chief Drainage Channel Reclamation Work Plan Comments (April 2011 <br />version), Permit No. M-1981-185/CN-0 L" <br />NOTE: On May 20, 2011, the Division received a draft version of the plan referenced in <br />this adequacy item. The draft version was submitted electronically, as an attachment to <br />an email, which was not followed-up with an official submittal to the Division. The <br />6/6/2011 <br />3
