Everist (LGE) continued minina in the Fort Lupton Sand and Gravel mine phase known as the
Hill-Oakley phase in 2010 and 2011. This phase may be mined more throughout the next year,
because an oil well located in this phase may be capped and removed, allowing LGE to mine more of
the natural sand and gravel deposit. LGE also mined to the floor of the deposit in the Fort Lupton
East phase. Reclamation - sloping and some seeding - will occur in both the Hill-Oakley and Fort
Lupton East phases later in this calendar vear. Please see the attached mar) for Fort Lupton Sand
and Gravel mining areas discussed above.
Much of the active mining operations of the past year were conducted - and in the coming year will be
conducted - in LGE's adjacent Lupton Meadows Reservoir (M-2002-104). These mining operations
will be discussed in more detail in that site's annual report. [For reference, Technical Revision (TR-5)
dated 01/25/2010 approved connecting the two mine operations and the related transfer of materials.)
Please show the location of the acreage for items with a * on your map. Indicate the phases of the reclamation which have
been completed, correlated with your timetable. Phased operations: show dates extraction ceased & dates reclamation began.
NOTE: If there have not been any changes since the last annual report and you previously submitted a map which correctly
depicts the current acreage in items 4 through 10, then a map is unnecessary. However, this must be stated above.

Date: oX? b ?0
Please supply current contact name, company, mailing address, phone & fax numbers, and Federal ID below:
Contact Name & Title: Lynn Mayer Shults, Regulatory Manager
Company and Address: L.
