PERMIT #: M-1977-285 <br />INSPECTOR'S INITIALS: RCO <br />INSPECTION DATE: April 20, 2011 <br />OBSERVATIONS <br />This was a routine inspection performed by the Division as part of its monitoring of 112d permits, and also to <br />inspect the proposed location of an Environmental Protection Facility: the lined ore pad. The site was not <br />active on the date of the inspection. The entrance gate was locked, the buildings were secured, the portal was <br />backfilled and secured with a locked grate, fluids and ore were removed from the surface, and all equipment <br />was gone. <br />Backfilling and Grading: <br />The portal was backfilled as a precaution against unauthorized entry during the period that the site is inactive. <br />The pad and portal area has been graded to be stable for the short-term, but final grading has not been <br />performed. <br />The grading at the GMG #13 pad area has been maintained to contain stormwater. The portal to the GMG <br />decline has been partially backfilled to prevent unauthorized entry, but this is not the final backfill and <br />grading. <br />Vent shaft site S-5 has been backfilled and closed permanently. No settling was observed, and no <br />revegetation was apparent. <br />Complete Inspection: <br />All pad, dump, and portal areas were inspected. Most vent shaft and power drop locations were inspected. <br />Several shafts contained new vent fans on the same concrete collar, and possibly also contained upgraded <br />electrical supply panels. <br />Financial Warranty: <br />The operator performed a certain amount of clean-up in 2009 to prepare the site for inactivity. The bond is <br />due for a recalculation as part of finalizing the current amendment (AM-3) but the present bond amount is <br />considered sufficient at this time. <br />Gen. Compliance With Mine Plan: <br />Portions of the top surface of the large waste rock dump ( in the western end of the site) has been raised <br />during the past episode of mining. It is here that the lined ore pad will be installed, and the location looks <br />appropriate. <br />Revegetation: <br />The operator has monitored and treated noxious weeds, including Halogeton, more during the past few years. <br />The amount of Halogeton observed on the day of this inspection was far less than the amount observed <br />during past inspections. No other noxious weeds were observed. No revegetation seeding has been carried <br />out at this time. <br />Sediment Control: <br />The sediment and runoff control berms below the dump and along the road were intact. the catchment ponds <br />appeared to be cleaned out and capable of full capacity. The dam for the runoff control pond east of the main <br />portal was in good condition. <br />Page 2 of 5
