C052154-08524 <br />Pursuant to Title 40 of the Code of Federal Regulations, Section 144.41(d) and Part III(B) <br />of the above referenced Class 11 Underground Injection Control (UIC) permit, a modification to <br />this permit is made. Changes made to the modified version of the PART lI. SPECIFIC PERMIT <br />CONDITIONS (Pages 6 and 9) are as follows: <br />Original version PART II, Section A.3 (Page 6) <br /> <br />(f) one-half (1/2) inch female iron pipe fitting, isolated by shut-off valves and located at the <br />wellhead at a conveniently accessible location, for the attachment of a pressure gauge <br />capable of monitoring pressures ranging from normal operating pressures up to the MAIR <br />(i) on the 7" tubing - 9 5/8" casing annulus, and <br />(ii) on the'annulus between T' tubing and three smaller injection and production <br />tubings, and <br />(iii) on the surface reflex tubing; and <br /> <br />is modified to read: <br /> <br />(f) one-half (1/2) inch'', female iron pipe fitting, isolated by shut-off valves and located near <br />the wellhead prior to any process activities, at a conveniently accessible location, for the <br />attachment of a pressure gauge capable of monitoring pressures ranging from normal <br />operating pressures up to the MAIR <br />(i) on the, 7" tubing -'? 5/8" casing annulus, and <br />(ii) on the annulus between T' tubing and three smaller injection and production <br />tubings, and <br />(iii) on the surface reflux tubing; and <br />
