The Water Quality Control Division will reissue <br />the permit before that expiration date. <br />Our records indicate that you are currently covered by this permit for stormwater discharges from the above- <br />referenced facility. In order to maintain coverage for this permit past September 30, 2007, you must complete and <br />return the enclosed Reapplication and Record Verification form. This will constitute reapplication for the reissued <br />permit. <br />On the attached Reapplication and Record Verification form you will find the latest information that we have in our <br />records for your facility. Please review the information carefully, make whatever changes are necessary, sign and <br />return it to our office by June 30, 2007. In addition, if available, please provide the email addresses requested in <br />items A and B on the form. An addressed envelope has been included. (Our address is also at the top of the form.) <br />Note that by signing the Reapplication and Record Verification form you are also certifying that an up-to-date <br />Stormwater Management Plan (SWMP) is being implemented at your facility. Therefore, you must ensure that the <br />SWMP in place for your facility is being maintained to accurately reflect potential pollutant sources and <br />management practices at the site. Refer to Item 4 of the Q&A. <br />Please refer to the Q&A section on the back of this letter for answers to other commonly asked questions.
