UA - Onshore Gas Number: CO-HSE-032 <br /> HSE Procedure Custodian: Colorado Operations <br /> Spill Prevention, Control and Revision: 0 <br /> Countermeasure (SPCC) Plan Effective: January 2012 <br />4.0 SPILL NOTIFICATION and REPORTING PROCEDURE <br />4.1 Internal Notification and Reporting <br />Refer to the Mahogany Research Project's Emergency Response Plan for requirements related <br />to internal notification and reporting, i.e. within Shell companies. <br />4.2 External Notification and Reporting <br />In the event of a reportable oil spill or unplanned discharge that impacts or threatens to impact <br />surface water, a dry gully, or ground water, external notification, both verbal and written, is <br />required according to Table 4.2. <br />Telephone numbers are summarized in Table 4.3. <br />Before notification and or reporting: <br />• Verify information before communicating it. <br />• Never speculate as to the cause of an incident or make any acknowledgment of <br />liability. <br />Do not delay initial notification, verbal or written, due to incomplete information. <br />The applicable Shell incident notification or reporting form should be utilized to document <br />factual information related to the spill event, and should as a minimum capture the following <br />information in a report: <br />Continued on next page <br />Printed: April 7, 2011 Section 4 Page 19 of 71 <br />The controlled version of this 'controlling document' resides online in Livelink®.
