May 2, 2011 C- 1981 - 020 /Munger Canyon Mine MPB <br />When we arrived on -site the locked access gate was open and we noted that the adjacent surface property owner <br />(# 11 enterprises) was doing some maintenance work on their property. The access road to the mine site was dry <br />and the road was maintained and in good condition. Culverts were clear of debris and functional. The large 72" <br />culvert located at the Munger Creek crossing is shown in Photograph No. 9. No problems were noted along the <br />length of the road up to the former portal bench access road. <br />REVEGETATION — Rule 4.15 <br />Vegetative Cover; Timing: <br />The vegetation along the reclaimed portal access is becoming well established between the riprap drainages and at <br />the portal backfill area. A portion of the reclaimed access road is shown in Photograph No. 10. Vegetation is also <br />well established at all other mine site locations including the waste disposal site, cover soil stockpile site, and both <br />topsoil stockpile locations. The entire mine site remains stable and well maintained with no problems identified <br />during this complete inspection. <br />SIGNS AND MARKERS — Rule 4.02: <br />The Mine and Permit identification sign was properly displayed and maintained at the entrance to the permit area <br />from Highway 139. Stream Buffer Zone signs were in place along Munger Creek. <br />OFFSITE SUPPORT FACILIITES - Rule 4.04,4.28: <br />The reclaimed Loma Loadout site was inspected. The sign with operator information, DRMS Permit Number and <br />MSHA ID is in good condition and clearly visible at the site.
